Transfer of a Student with a Disability Enrollment Process
- If a student meets the criteria to enroll in DPSCD the Membership Secretary

- Non-Resident students in the following programs
- Early Childhood
- Mild Cognitive Impaired (Except Highland Park Grades 9-12 for the reminder of the 24-25 SY)
- Non -Resident ACT 18 Students without approval from Executive Director
- ALL ACT 18 Enrollments for the following districts will be processed by the ESE Enrollment Office/Enrollment Specialist
- Qualifying Districts with approval

- ACT 18 Enrollments for the following district procedure:

- Catchment District Liaison submit request to Executive Director for ACT 18
- Executive Director for Act 18 forwards approval or denial to ESE Enrollment Office
- ESE Enrollment Specialist will transfer student into appropriate school
- ESE Enrollment Specialist will communicate:

- On the date of enrollment, the Receiving School will:

- Membership Secretary will Enroll the student
- Membership Secretary will Complete the Transfer of a Student with a Disability Form in PSSP
- ESE Lead teacher or Designee will complete the Notification of an Out of District Transfer of Student with Disability Part B ESE Enrollment and send to the Membership Secretary
- Membership Secretary will email the Transfer of a Student with a Disability Form in PSSP and Out of District Transfer of Student with Disability Part B ESE Enrollment to ese.specialprograms@detroitk12.org
- Membership Secretary will inform all other non-residents to enroll with resident district
- Membership Secretary will Assist parents in completing the Enrollment Packet
- Membership Secretary will complete the ESE Intake Form
- Membership Secretary will complete record request and document (FLAG: Documentation PSSIS

- Membership Secretary will Request Unique Identification Code (UIC)
- ESE Lead Teacher or Designee will Import Student into PowerSchool Special Program (PSSP)
- ESE Lead Teacher or Designee will Upload ESE documents into PSSP
- ESE Lead Teacher or Designee will Generate and complete the Transfer of a Student with a Disability in (PSSP)
- ESE Lead Teacher in consultation with the parent offer FAPE immediately upon receiving the IEP.

- ESE Lead Teacher or Designee will send the Out of District Transfer of a Student with Disability Part B ESE Intake form to esespecial.programs@detroitk12.org
- ESE Lead Teacher or Designee will Process one time consent and parent notification
- ESE Lead Teacher or Designee Obtain Signature for FAPE and One Time Consent

- We have knowledge that a scholar is eligible for programs and services, but we do not have the documentation

- Process of Re-enrolling a DPSCD student (This is not a Transfer of a Student with a Disability):
The following will take place on the date of enrollment:

- The School Membership Secretary will determine if the student never attended another school since attending DPSCD
-
- If the IEP is current, the Membership Secretary will complete ESE Intake Form; Membership Secretary will email ESE Intake Form esespecial.programs@detroitk12.org

- IEP is expired or the program is no longer applicable, the Membership Secretary will complete the ESE Intake form and email to esespecial.programs@detroitk12.org and

- ESE lead teacher or Designee will assign case manager, the case manager will complete an IEP within 5 school days
- Documenting Transportation for Non-Resident Act 18 Programs

- When a non-residential student enrolls in an ACT 18 Program, the student’s LEA/PSA is responsible for the provision of specialized transportation services.

ESE Records Request Process
IMPORTANT: The process of requesting records should be expedited if the student arrives to enroll at your school without a copy of the IEP so that we adhere to the timeline of offering FAPE
Complete the following steps when requesting records for a student that enrolls from out of district:
- Attempt to obtain a copy of the most recent IEP from the parent/guardian upon enrollment
Follow the Records Request Process
- Staff make records request immediately upon enrollment
- Staff follow-up within 1 week if records not received
- Staff follow-up again on a weekly basis if records not received
- After 30 days, refer to SBET
- If a parent states, the student enrolling has an IEP, but they don’t have the document:
- See special circumstances(add link to external hub) if IEP records are not available (refer to Transfer of a Student with a Disability Enrollment Process document)
- Email the previous school of attendance to request records using the district ESE Document Request form.

- CC: the ESE Lead Teacher on all email communication
- Follow-up one week later if no records received
- Make follow-up phone calls as needed to request records from the previous school of attendance

- Psychological Testing Results
- MET/ER for Eligibility
- Re-Evaluation IEP (most recent)
- Signature Pages
- Placement Consent/Consent for Services
- Pertinent Medical Information
- RTI/SST Data
- 504 Plan
- If the records are not received within 30 school days, the student should be referred to the SBET team.
